Verification of Experience Request Form
The state requires at least 3 years of qualified teaching experience. We require a Verification of Experience Form be completed by an administrator confirming you meet this requirement.

This automatic email with the electronic form sometimes winds up in junk folders so make sure your administrator knows it is coming their way.

Please provide the name and contact information for a school or district administrator who can verify that you have successfully completed the prerequisite three (3) years of full-time employment within a school district in order to be admitted to the program. 
Upon submission of this request, a Verification of Experience form will be sent electronically to the school or district administrator provided above.
